Our caring, experienced Certified Diabetes Educators will
help you learn how to self-manage your diabetes following
the Standard of Care guidelines provided by the American Diabetes
Association. Working with your doctor, our team will assess
your needs and provide a care plan which may include all or
some of the below:
Diabetes Self-Management Training
- Blood sugar monitoring (glucose meter will be provided)
- Oral & Insulin management skills
- Nutrition education & meal planning
- Cholesterol & high blood pressure management
- Exercise & weight loss counseling
- Stress management
- Preventing or delaying long-term complications
Sweet Success Program
Diabetes counseling during pregnancy for women with existing
diabetes or newly diagnosed with Gestational diabetes. The
individual receives education on nutrition, exercise and glucose
monitoring.
Pre-diabetes Class
Here, we focus on preventing the diagnosis of Type 2 diabetes
through healthy eating, weight loss and exercise. In addition,
cholesterol management is discussed. Recent research has shown
that as little as a 57% weight loss and 150 minutes/week
of exercise can return blood sugars to normal ranges!
Included in all of the above are follow-up sessions and a
monthly support group to provide ongoing support for the daily
management of your diabetes.
